Patient blood management inside an enhanced recovery pathway

The challenge

Preoperative anaemia is common and is usually found too late to treat properly. Blood loss and transfusion around surgery are associated with longer stays, more complications and more readmissions.

The people who could prevent that — surgeons, anaesthesiologists, ERAS nurses, haemovigilance nurses, clinical biology, allied health — work from separate systems and largely on paper. Nobody has a live view of where a patient is in the pathway, and nobody sees the outcome data until long afterwards.

What we did

A digital care pathway covering the whole surgical journey, from the preoperative outpatient visit through the hospital stay to recovery at home, combining enhanced recovery after surgery with patient blood management.

Early risk stratification. Laboratory integration detects anaemia early and flags high-risk patients automatically, triggering iron therapy and prehabilitation rather than leaving it to be noticed.

One shared pathway. All disciplines work from the same record, with escalation logic that routes an alert to first, second or third line depending on what it is.

Patient preparation. Digital education and checklists replace paper booklets, before and after surgery.

Post-discharge monitoring. Remote follow-up with threshold-based logic to catch delayed recovery, and wearable integration for objective activity data.

Live dashboards. Automatic capture from the hospital record, the laboratory system and the national telemonitoring hub feeds clinical and operational indicators, so the pathway can be benchmarked and improved rather than assumed to work.
